- On June 2, 2008, the ' Northwest Family Care Collaborative ' of 11 counties became a new 11 county Long Term Care District

- The NW Long Term Care District (NW-LTCD) is a separate unit of government, created by the counties but operating entirely independently of the counties
- 'NorthernBridges' ~ a new, publicly owned Managed Care Organization (MCO) ~ will operate Family Care under the auspices of the NW-LTCD
- In May 2009, the NW-LTC District will begin providing the Family Care Benefit to eligible adults who reside in the 11 county region and choose to enroll as members of NorthernBridges
- Within two years, over 3,000 people will be served and waiting lists will be eliminated
- A 16 member Board of Directors is guiding the NWLTC District and the creation of the new MCO
- A region-wide network of four Aging & Disability Resource Centers (ADRC) is also being developed. ADRCs offer one-stop-shop information / assistance and determine an individual's eligibility for Family Care
- The ADRCs will work hand-in hand with NorthernBridges but will be totally separate, independent entities
This website is one way consumers and other 'stakeholders' can learn of what is going on throughout the planning/implementation process. As an accessible repository of pertinent information/ materials, it will continue to grow with daily updates throughout the transition.
